13.2 Duty to Furnish Information

Every Participant and beneficiary is required to furnish the Plan Office with any information or proof in support of dates of birth and marriage and and any other information reasonably required by the Plan Office to administer the Plan. If such information is not supplied promptly and completely, there are likely to be unnecessary delays in the processing of benefit payments. The Board has the power to deny, suspend or discontinue benefits until requested information is provided.
