General

  • Pension Application Request Form – Use this form to request application and documents to commence pension benefits.
  • Direct Electronic Deposit Request Form – Use this form to apply for a Direct Deposit for the Defined Benefit Plan.
  • Withholding Certificate for Pension or Annuity Payments (W-4P) – Include this form with the Withholding Certificate for Pension or Annuity Payments if you elected to have Income tax withheld from your pension or annuity.
  • Withholding Certificate for Pension or Annuity Payments (DE 4P) – Unless you elect otherwise, the law requires that personal income tax be withheld from payments of pensions and annuities. Use this form to elect whether income tax will be withheld from pension payments.
  • Retiree Health & Welfare Monthly Charges - Deduction Assignment – As a convenience to retirees who are required to make monthly direct payments to the San Francisco Electrical Workers Health and Welfare Plan, the Trustees of the Northern California Electrical Workers Pension Trust have amended the Plan to allow an "after tax" and "voluntary" deduction from monthly pension checks. If you are interested in this option, please complete the following and return to the Plan Office.
  • Defined Benefit Designation of Beneficiary Form – This form is used to designate primary or contingent beneficiaries for the Defined Benefit Plan.
  • New $300,000 Dollar Limit on Pension Enhancement – Under the Pension Plan's pension enhancement rule, a retiring participant may elect to transfer some or all of the participant's SFEW Retirement Savings Plan to the Pension Plan for the purpose of increasing the participant's guaranteed monthly pension benefit.
  • Pension Enhancement Option – Retiring Participants who are contemplating receiving a distribution from their Individual Retirement Savings Individual Account in one of the monthly installment arrangements, or as a lifetime annuity, to help offset Early Retiree self-payments or other expenses may want to consider this option as an alternative. Before making a decision, you should carefully review the following information to determine if you are interested in the pension enhancement option.
  • Suspension of Benefit Plan Provisions – At the time of your retirement, you will be notified of the provisions of the Plan relating to prohibited employment, overpayment, recovery of overpayment and the procedure you would follow to secure a review of any monthly pension payments that have been suspended. In general, your benefits will be suspended for any period in which you engage in "prohibited employment" as defined under the Plan.
  • Rules Regarding Minimum Distributions and Distributions After Death – The Internal Revenue Code requires the Plan to apply these "minimum distribution" rules, and this Explanation summarizes how they operate.

Notices

  • Pension Plan Overview - April 8, 2010 – This notice is a summary of the NCEW Pension Plan January 16, 2009 meeting, describing changes to the Plan that have taken effect since the previous meeting, Plan demographics, an overview of the current Plan status, and a legislative update.

QDRO

  • Procedures for Administering Domestic Relations Orders (QDROs) – Pursuant to the Employee Retirement Income Security Act, as amended ("ERISA"), the Board of Trustees of the Northern California Electrical Workers ("NCEW") Pension Trust adopts these procedures for administering domestic relations orders which affect the pension benefits of Participants in the NCEW Pension Plan and/or the NCEW Retirement Savings Plan (Profit Sharing Plan).
  • Sample QDRO – This sample order is intended to assist attorneys in preparing Qualified Domestic Relations Orders (QDROs). The language in this sample order is currently acceptable to the Plans as meeting the requirements of a QDRO, although the language has not been challenged in court nor has a court definitively stated that the language meets ERISA's minimum standards for a QDRO.
