$50,000 ($100,000 for accidental deaths). Health and Welfare coverage must be in force at the time of death. The death benefit does not apply to deaths occurring as a result of suicide or self-inflicted injuries, or upon the death of pre-age 62 retirees who are covered under the Plan as a result of permanent disability.
Active participants who satisfy the minimum hours worked rule are eligible. Application and proof of inability to work satisfactory to the Plan is required.
To satisfy this requirement, an applicant must meet at least one of the following employment conditions:
Participants having less than 36 months may qualify for one month of disability benefits for each calendar month they worked a minimum of 120 hours (or were available for work and registered as such with IBEW Local 6) within the 36 months prior to disability.
Disability benefits are limited to the number of months the Participant was employed for at least 120 hours (112 hours during periods when a 28 hour work week is in force) during the 36-months immediately preceding the onset of disability up to a maximum of:
If earlier, benefits will cease upon recovery or retirement under an IBEW Local Pension (including Defined Contribution and Defined Benefit Plans)
$1,000 per month (first 6 months, FICA taxes will be withheld. Benefits are prorated for any month in which the participant is not disabled for the entire month. In addition, $31.25 per month (or prated amount) is payable to the Northern California Electrical Workers Pension Plan. Exception: the pension contribution ceases upon retirement under the Northern California Electrical Workers Pension Plan.
The first check is payable at the end of the month following a 30 day waiting period, and will resume, thereafter, at the end of each month during period of entitlement.
Application must be filed no later than 90 days after the later of 1) the end of the 30-day waiting period or 2) exhaustion of your health & welfare reserve hourbank balance.
