If a Dependent loses, or will lose, Plan coverage due to the event of divorce, legal separation or ceasing to be a Dependent, such Dependent or the covered Employee must notify the Plan Office within 60 days of the event in the manner prescribed by the Plan's COBRA notice procedures. Failure to provide timely notice will result in a termination of the Dependent's COBRA rights under the Plan. A Dependent must notify the Plan Office of the birth, adoption or placement for adoption of a child while receiving COBRA Coverage, also as required by the Plan's COBRA notice procedures.
