Life Event - Moving

It is highly important that you notify the Plan Office of your new address as soon as you move.

Plan Checklist

• If you are covered by an HMO, you need to check whether if you have moved outside the designated service area, and if so, what coverage options are open to you.
• It is your responsibility to notify the Plan Office when you move. If you fail to do so, you may not receive important communications regarding your benefits.
